$150/Month (In-person or telehealth visit each month, if desired, and unlimited messaging.)

$75/month for Adult membership (4 visits a year, no provider messaging )

Once a year adult wellness visit without membership

We do strongly encourage that each patient have a separate primary care provider for acute care needs that may arise. The primary focus of our office is wellness and chronic care management. We cannot guarantee availability for acute visits.
*Disclaimer: Monthly memberships are based on a 6 month commitment to our practice. If you decide to cancel your membership after at least 6 months you will need to cancel through your account at daytonfm.com. It will not automatically stop when 6 months are completed. If monthly membership is canceled prematurely membership cannot be reactivated for 6 months.
